1. Services Description

We offer a Simple Rate Box program for the compliant destruction of unused, damaged, expired, or outdated pharmaceuticals (including DEA Schedules II-V controlled substances, Rx-only, and OTC items). You purchase a flat-rate prepaid USPS Priority Mail Flat Rate Box service. You are responsible for:

Packing and shipping your own pharmaceuticals to us using USPS Priority Mail (we do not ship products to you).
Uploading required documents (DEA registration, state license, client registration form) for account verification and DEA compliance.
Completing accurate inventory forms.

Upon receipt, we verify contents, provide DEA-compliant documentation (transfer records, Certificate of Destruction), and destroy the materials via approved incineration within DEA-required timelines (no later than 30 days after receipt). Services are provided on a one-time or repeat-order basis with no long-term contracts

2. Eligibility and Account Setup

You must be a licensed DEA registrant (or authorized representative of one) eligible to possess and transfer controlled substances. By creating an account, you represent that you have authority to bind your organization and that all uploaded documents are accurate and current. We reserve the right to refuse service or terminate accounts for non-compliance.

3. Ordering, Payment, and No-Refund Policy

Purchases are made through WooCommerce on the Site.
Payment is due in full at the time of purchase.
Once you ship your pharmaceuticals to us, the sale is final. There are no refunds, cancellations, or returns. By shipping, you agree that you have inspected the box contents, accept full responsibility for proper packaging and compliance with all laws, and authorize us to destroy the materials as described.
You remain responsible for all shipping costs and any additional charges if your shipment exceeds weight limits or contains non-conforming items.

4. Shipping and Customer Responsibilities

You must ship only eligible pharmaceutical waste in the purchased Simple Rate Box using USPS Priority Mail.
You are solely responsible for proper packaging, labeling, and compliance with USPS, DOT, DEA, EPA, and all other applicable federal, state, and local regulations.
We are not liable for loss, damage, or delay during transit.
Non-conforming waste (e.g., sharps, hazardous chemicals not covered by the program, or improperly packed items) may result in additional fees or return at your expense.

5. Our Obligations and Limitations

We will:

Handle and destroy materials in compliance with DEA reverse distributor rules (21 CFR Part 1317).
Provide required documentation for your records.

We do not guarantee credit returns from manufacturers or any specific destruction timeline beyond regulatory minimums. Services are provided “as is” without warranties of any kind.

6. Limitation of Liability

To the fullest extent permitted by law, we are not liable for any indirect, incidental, consequential, or punitive damages arising from your use of the Services or Site. Our total liability shall not exceed the amount you paid for the specific Simple Rate Box order. You agree to indemnify and hold us harmless from any claims, losses, or liabilities resulting from your violation of these Terms or applicable laws.

7. Intellectual Property and Site Use

The Site content is our property. You may not copy, modify, or use it for commercial purposes without permission.

8. Termination and Changes

We may terminate or suspend your account for violation of these Terms or legal requirements. We may update these Terms; continued use after changes constitutes acceptance.

9. Governing Law

These Terms are governed by the laws of the State of Arizona, without regard to conflict of laws principles. Any disputes shall be resolved exclusively in the courts of Maricopa County, Arizona.
