Medium Box Controlled Substance Disposal Service
The Medium Box Destruction Service gives DEA registrants more capacity for routine clean-outs of unused, expired, or damaged pharmaceuticals. Everything ships together in one USPS Priority Mail medium box (approx. 11” x8.5 ” x 5.5”).
You complete the inventory form for what’s in your box and keep a copy for your records. Once your box arrives, our team verifies the contents against your forms and takes care of witnessing, documentation, and DEA reporting from there. No DEA Form 41 required. No witnesses needed. No waste haulers to coordinate.
Simply pack your box and ship it prepaid via USPS Priority Mail.
